PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Care Coordination Services team as the CalAim Case Manager at the Santa Ana, CA office.
ABOUT PATH
Since its foundation in 1984, PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.
ABOUT OUR TEAM
PATH’s Health Programs dept provides services to participants receiving services through managed care plans. Through these partnerships, staff provides support to eligible Medi-Cal beneficiaries experiencing homelessness, including chronic homelessness, and who have at least one complex physical, behavioral or developmental health need with the goal of improving health outcomes and/or decreasing utilization of high-cost services. The program provides the full range of outreach and engagement, assessment and care management, coordination of care, health promotions, coordination of transitions in care, and family supports. Additionally, these programs are focused on connecting participants to appropriate permanent housing linkages with the hope that retention is increased due to stable medical care coordination.
ABOUT THE JOB
The role of the Case Manager under CalAIM's Community Support program will focus on member housing navigation and retention and work with other team members to ensure the medical and housing stability of each participant.
