PATH is dedicated to helping individuals, families and communities end their homelessness.

PATH SANTA CLARA
PATH began serving Santa Clara County in 2015, initially aiding unsheltered individuals in downtown San Jose. Since then, PATH has expanded its outreach services into a multi-disciplinary team and also launched an emergency interim housing community for families.
PATH provides onsite, wrap-around supportive services to formerly-homeless residents in permanent supportive housing and field-based supportive services to vulnerable renters. All programs are designed to end homelessness in Santa Clara County so participants can rebuild their lives and move into the permanent homes we all deserve.
