PATH provides the support that homeless individuals and families need in order to successfully transition from living on the street, or in shelters, to thriving in homes of their own. High-quality supportive services are critical to ensuring that the people we serve are able to not only move into permanent homes, but also stay in their new homes long-term. The services we provide are available to individuals, families, and Veterans who are homeless or at risk of becoming homeless.
The individuals we serve have an array of unique experiences and we aim to tailor our service delivery to be able to address each person’s goals to make it home.

PATH outreach teams work throughout the region, targeting areas identified as homeless “hot spots” and engaging individuals, Veterans, and families currently living on the street.

PATH Ventures, PATH’s housing development arm, builds and operates affordable rental apartments with supportive services. PATH also provides supportive services to more than 1,500 individuals living in permanent supportive housing throughout the community.
