PATH provides the support that homeless individuals and families need in order to successfully transition from living on the street, or in shelters, to thriving in homes of their own. High-quality supportive services are critical to ensuring that the people we serve are able to not only move into permanent homes, but also stay in their new homes long-term. The services we provide are available to individuals, families, and Veterans who are homeless or at risk of becoming homeless.
The individuals we serve have an array of unique experiences and we aim to tailor our service delivery to be able to address each person’s goals to make it home.
Services
PATH employment programs work with individuals and Veterans to help them increase their job skills and find meaningful employment.
PATH outreach teams work throughout the region, targeting areas identified as homeless “hot spots” and engaging individuals, Veterans, and families currently living on the street.
PATH provides eviction prevention assistance funding for individuals, Veterans and families at-risk of homelessness.
PATH’s housing navigation services assist individuals, families, and Veterans to find and secure a permanent home.
PATH interim housing provides transitional housing and recuperative care to individuals, families, and Veterans experiencing homelessness.
PATH rapid rehousing programs assist individuals, families, and Veterans with finding permanent housing by providing short term rental assistance, supportive services, and case management.
